Publicly Funded Immunization Schedule for Ontario

2 Months of Age

  • Diphtheria, Pertussis, Tetanus, Polio, HIB
  • Pneumococcal Disease (pneumonia)
  • Rotavirus (oral vaccine)

4 Months of Age

  • Diphtheria, Pertussis, Tetanus, Polio, HIB
  • Pneumococcal Disease (pneumonia)
  • Rotavirus (oral vaccine)

6 Months of Age

  • Diphtheria, Pertussis, Tetanus, Polio, HIB

IMPORTANT: Seasonal influenza is recommended for all individuals 6 months of age and older.

12 Months of Age

  • Measles, Mumps, Rubella
  • Meningococcal Disease (Men C-C)
  • Pneumococcal Disease (pneumonia)

IMPORTANT: The measles, mumps, rubella vaccine and the meningococcal vaccine must be given AFTER the first birthday.

15 Months of Age

  • Varicella (chicken pox)

18 Months of Age

  • Diphtheria, Pertussis, Tetanus, Polio, HIB

4-6 Years of Age

  • Diphtheria, Tetanus, Pertussis, Polio
  • Measles, Mumps, Rubella, Varicella (chicken pox)

Grade 7

  • Hepatitis B
  • Meningococcal Disease (Men-C-ACYW-135)
  • HPV-9 (human papillomavirus)

Adolescent Booster

  • Tetanus, Diphtheria, Pertussis

Adult Immunizations

  • Tetanus, Diphtheria, Pertussis
  • Pneumococcal Disease (65 years of age)
  • Shingles (65-70 years of age)
  • Influenza
