Getting your child vaccinated is an important decision. When you choose to vaccinate your child, you are protecting them from many vaccine preventable diseases (Maladies pouvant être prévenues par la vaccination) that can cause serious illness and even death.

Please bring a copy of your child's vaccination record along with the completed Child Care Registration Form below. Fax, email, mail or drop off the completed form and vaccination record to the Health Unit.

Please provide a copy of your vaccination record along with the Health Assessment Form below. Fax, email, mail or drop off the completed form to the Health Unit.

If you plan to bring a child other than your own to receive their vaccines at the Health Unit, proof of legal guardianship/permission will be requested by the immunizing public health nurse. Please remember to bring this documentation with you to every appointment.
