School Vaccine Information
Keeping school-aged children up to date with recommended immunizations helps protect their health and reduce the spread of vaccine-preventable diseases in the community.
Under Ontario's Immunization of School Pupils Act (ISPA), students under 18 attending elementary or secondary school must be immunized against designated diseases or have a valid exemption on file. Parents or guardians are responsible for ensuring their child's immunization record is up to date with the Health Unit.
The following vaccines are required under the ISPA for all children who attend primary or secondary school:
- diphtheria
- tetanus
- polio
- measles
- mumps
- rubella
- meningitis (meningococcal disease)
- whooping cough (pertussis)
- chickenpox (varicella) — required for children born in 2010 or later
Optional (Recommended in Grade 7):
Please note: Hepatitis B and HPV vaccines are optional. Exemptions do not apply to these vaccines, and your child will not be suspended for not receiving them. You may continue to receive notices about these vaccines until your child graduates from high school.

To have your child vaccinated at school, please complete the consent form provided by your child’s school (e.g., through Edsby, email, or the school’s usual method of communication).
Please complete the form even if you do not want your child to be vaccinated. You can select “No” on the consent form.
The information you provide is sent to the Health Unit to help determine which students can be vaccinated. Completing the form in advance helps the clinic run smoothly and reduces the risk of potential errors.
Please note: Under section 4(1) in the Health Care Consent Act, if parental consent is not provided, a student may be able to consent to vaccination themselves if they are capable of understanding the benefits and risks of vaccination.

If your child is vaccinated by a family healthcare provider, please ensure their immunization records are reported to the Health Unit so we can maintain an up-to-date immunization record. Records can be:
- Emailed to vpd@healthunit.ca
- Faxed by your healthcare provider to 705-474-9399
- Reported through our ICON immunization record reporting system. Visit our Immunization Records page to learn more.
Vaccine Exemptions
Parents/guardians who intend to submit a vaccination exemption with respect to the vaccination requirements under the ISPA, will need to complete and submit the original Statement of Conscience or Religious Belief Affidavit or Medical Exemption to the Health Unit’s VPD Program.
Medical exemption: Must be completed and signed by a legally qualified medical practitioner.
Religious or Philosophical Exemption
If you wish to have your child exempted from required immunizations due to a conscience or religious belief, you must complete the following steps through the Health Unit.
1. Complete an education session
Contact the Health Unit to let us know that you wish to have your child exempted from the required immunizations.
Health Unit nurses will:
- Advise you of the steps required to complete a valid exemption, including watching the vaccine education video.
- Provide you with a signed and dated Vaccine Education Certificate after you have watched the video.
2. Complete a form
Complete a Statement of Conscience or Religious Belief form.
Once completed, the form must be signed by a commissioner for taking affidavits in Ontario.
3. Make copies for your records
Make copies of the following documents:
- Your Vaccine Education Certificate
- Your signed Statement of Conscience or Religious Belief form
You will need to submit the original documents to the Health Unit.
Please note: It is important to keep copies for your records, as the Ministry and local public health units do not keep copies of exemption documents.
4. Submit the original documents
Submit the original Vaccine Education Certificate and signed Statement of Conscience or Religious Belief form to the Health Unit. Documents may be submitted in person, by mail, or by email

Keep in Mind: During an outbreak of a vaccine preventable disease, a child with a vaccination exemption may not be permitted to attend the school until the outbreak is declared over by the Medical Officer of Health.
