Vaccination is a safe and effective way to protect children and adults from vaccine-preventable diseases and their complications. Keeping vaccination records up to date helps support a healthy environment for children, staff and volunteers.

Vaccination Assessment Forms for Children

If you are registering your child for child care, complete the following form. Once completed, the child care centre will submit the form directly to the Health Unit’s Vaccine Preventable Diseases (VPD) Program assessment.

Child care operators are responsible for collecting and securely maintaining vaccination records and any applicable exemptions on file at the centre.

Please note that the Health Unit can only accept and process these forms when they are submitted directly by the child care centre. Forms submitted by parents or guardians cannot be processed.

Vaccination Assessment Forms for Staff/Volunteers

If you are a staff member or volunteer at a child care centre, complete the following form and, where applicable, provide a copy of your vaccination record to the centre.

The child care centre must submit the form(s) directly to the Health Unit’s Vaccine Preventable Diseases (VPD) Program for assessment. Child care operators are responsible for collecting and securely maintaining vaccination records and any applicable exemptions on file at the centre.

Please note that the Health Unit will only accept and process forms submitted directly by the child care centre.

Vaccine Exemptions

Parents/guardians, staff members, or volunteers who intend to submit a vaccination exemption with respect to the vaccination requirements under the Childcare and Early Years Act, will need to complete and submit the original Statement of Conscience or Religious Belief Affidavit or Medical Exemption to the Health Unit’s VPD Program.

Medical Exemptions

Medical exemption: Must be completed and signed by a legally qualified medical practitioner.

Religious or Philosophical Exemption

Fill out a form
- Complete a Statement of Conscience or Religious Belief form.
- Once it’s complete, you must get it signed by a commissioner for taking affidavits in Ontario.
Make copies for your records
- Make copies of your signed Statement of Conscience or Religious Belief form
- You will need to submit the original versions.
- It is important that you keep copies for your records because the ministry and local public health units do not keep your exemption documents
Submit the originals
- Submit the original copies of your signed Statement of Conscience or Religious Belief form to the health unit in person, by mail, or by email

Keep in Mind: During an outbreak of a vaccine preventable disease, a child with a vaccination exemption may not be permitted to attend the child care centre until the outbreak is declared over by the Medical Officer of Health.
