How to Report Vaccinations

Open Immunization Connect Ontario (ICON)  and report a vaccination to the Health Unit or view your vaccination record.

The Immunization Connect Ontario service can be used to keep you and your family's vaccination records up-to-date. To use this service, you must have your Ontario Health Card, or Ontario Immunization ID (OIID) and set a PIN.

Once you are securely logged in, you can view your vaccination records and submit recent vaccines. There is also a printable schedule to help keep your vaccinations on time.

Please note: To ensure our vaccination information on file is accurate, a copy of the vaccination record or a paper/card from your health care provider must be attached when submitting the vaccination information through ICON. If this information is missing, we will be unable to process the vaccination information.

You can also report your vaccinations to us by:

• Dropping a copy of your record off at 345 Oak Street West, North Bay
• Fax your record to 705-474-9399
• Email a copy of your record to vpd@healthunit.ca
