4 Months

6 Months

  • Diphtheria, Pertussis, Tetanus, Polio, HIB

IMPORTANT: Seasonal influenza is recommended for all individuals 6 months of age and older.

12 Months

  • Measles, Mumps, Rubella
  • Meningococcal Disease (Men C-C)
  • Pneumococcal Disease (pneumonia)

IMPORTANT:  The measles, mumps, and rubella vaccine and the meningococcal vaccine must be given AFTER the first birthday.

15 Months

  • Varicella (chicken pox)

18 Months

  • Diphtheria, Pertussis, Tetanus, Polio, HIB

4-6 Years Old

  • Diphtheria. Tetanus, Pertussis, Polio

  • HPV-9 (human papillomavirus) 

Adolescent Booster
