The Flu shot is recommended for every person over 6 months of age and helps keeps you and your family protected throughout flu season.

Adults and children 5 years of age and older can also receive their flu shot at their local pharmacy! See what Pharmacies will be offering flu shots in your community.  Appointments can also be made at your family physican's office.

Fill out your flu consent form ahead of time

Adult Influenza Immunization Form ages 18 + (ENG ι FR
Child Influenza Immunization Form ages 6 months - 17 yrs old (ENG ι FR)

Proof of legal guardianship/permission will be requested by the immunizing Public Health Nurse when bringing a child other than your own to receive their flu vaccine.
